Job summary

Inova Fairfax Hospital is seeking an experienced Registered Nurse Case Manager I to join the Case Management Team. This full‑time, on‑site role offers Monday‑Friday scheduling with rotating weekends.

The RN Case Manager coordinates discharge planning and post‑acute care, collaborates with an interdisciplinary team, and drives utilization management to ensure efficient, patient‑centered care.

Qualifications

• Certification: Basic Life Support (BLS)
• Licensure: Licensed or eligible for licensure as a Registered Nurse in Virginia
• Experience: 1 year of case management and/or clinical care experience

Responsibilities

• Provides discharge planning and continuity of care for assigned patients in acute and post‑acute settings.
• Collaborates with the interdisciplinary healthcare team to coordinate post‑discharge services and transitions.
• Monitors utilization management, ensures regulatory compliance, and supports performance improvement initiatives.
