Inova Mt. Vernon Hospital is seeking an Experienced Registered Nurse Case Manager 1 to join the Case Management Team. This full-time role operates Monday–Friday with rotating weekends and focuses on discharge planning and care coordination for acute and post-acute patients.

Qualifications

• BLS certification required (American Heart Association).
• Licensed or eligible for Virginia RN licensure.
• 1 year of case management or clinical care experience.
• BSN required or ASN with a plan to complete BSN within 5 years and a documented timeline.

Responsibilities

• Provide discharge planning and continuity of care for acute and post-acute patients.
• Coordinate referrals to clinics, home health, hospice, SNF, acute rehab, LTAC, and medical equipment as indicated.
• Collaborate with interdisciplinary team to assess and coordinate discharge needs and post-discharge services.
• Document discharge planning information in medical records per department standards.
