Job summary

Inova Transitional Care is seeking an RN Case Manager to support Fairfax clinics, coordinating discharge planning and post-acute care for high-risk patients. The role emphasizes collaboration with physicians and the care team to ensure safe transitions from hospital to community settings.

The position requires a BSN with Virginia RN licensure and a minimum of 1 year in case management, with optional ADN BSN pathway. Monday–Friday, no weekends, primary location Fairfax.

Qualifications

• Education: BSN from an accredited school of nursing; ADN holders must complete BSN within 5 years of start date.
• Experience: Minimum 1 year of Case Management and/or Clinical Care experience.
• Certification: VirginiaRN license (or privilege under eNLC) and BLS for Healthcare Providers required on start.

Responsibilities

• Collects data for performance and outcome indicators and analyzes variances from plans of care with physicians and team.
• Provides discharge planning and continuity of care in acute and post-acute settings and facilitates referrals as indicated.
• Collaborates with interdisciplinary team, patients and families to coordinate discharge planning and post-discharge services.
• Acts as liaison between patients, families and healthcare team; supports social/financial resource referrals and advocates to remove barriers to care.
