Qualifications

• RN licensure and active registration.
• Minimum 1 year of nursing experience.

• Coordinate services and develop individualized care plans.
• Collaborate with physicians, nurses, and leaders to ensure safe transitions and clear communication.
• Document psychosocial assessments and discharge planning findings for the care team.
• Promote patient-centered care consistent with HCA Healthcare values.
