Job summary

Overland Park Regional Medical Center is seeking a Case Manager to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans reflecting each patient’s unique needs, partnering with physicians, nurses, and department leaders to ensure safe transitions and clear communication across the continuum of care.

Qualifications

• Associate Degree in Nursing required; Bachelor’s Degree preferred.
• Minimum 3+ years of clinical nursing experience.
• InterQual experience preferred.

Responsibilities

• Guide patients and families through orientation, discharge criteria and rights.
• Coordinate education for patients, families and caregivers on treatment planning.
• Develop and implement individualized care plans based on patient needs.
• Complete psychosocial assessments and document findings for the care team.
• Collaborate with Rehab Program Director and interdisciplinary team for seamless care.
• Promote values of compassion, respect, and excellence in patient interactions.
