Job summary

CVS Health is seeking a Complex Nurse Case Manager in Kentucky to assess, plan, implement and coordinate case management activities for members. You will work to improve health outcomes through proactive care planning, coordination with care partners, and effective use of resources.

The role requires an RN with KY license, 3+ years of clinical practice, and ability to travel up to 80-90% within the state. BSN is preferred; a comprehensive benefits package is provided.

Qualifications

• RN with current unrestricted KY licensure.
• 3+ years' clinical practice experience including patient care coordination and health assessments.
• Familiarity with MS Office Suite (Teams, Outlook, Word, Excel).
• Willingness and ability to travel in-state up to 80-90%.

Responsibilities

• Assess members through in-person or telephonic contact to assess, plan, implement and coordinate case management activities.
• Develop proactive care plans to address biopsychosocial needs and optimize health outcomes.
• Coordinate with care partners to ensure smooth transitions to programs and appropriate resource utilization.
• Review claims and determine impact on care planning and eligibility.
• Document assessments, care plans and progress in appropriate systems.
