Qualifications

• Active and unrestricted Compact Registered Nurse (RN) licensure in state of residence.
• 2+ years of experience in care management.
• 2+ years of Medicare experience.

Responsibilities

• Acts as liaison with members, families, employers, providers, and insurers.
• Implements and coordinates all case management activities for catastrophic and chronically ill members.
• Interacts telephonically or in person to assess needs and plan care.
• Develops care plans to optimize wellness and return-to-work or baseline function.
• Communicates with stakeholders and documents all case work activities.
• Collaborates with multidisciplinary team to maximize health outcomes.
