Job summary

Elevance Health is seeking a Nurse Case Manager II in Kentucky to manage care for members with complex needs. You will assess, develop, implement, coordinate, monitor and evaluate comprehensive care plans across the continuum.

The role may be conducted telephonically or on-site for discharge planning, with hybrid/virtual work and required in-person training sessions. BA/BS in health-related field and RN license are required; multi-state licensure is preferred.

Qualifications

• Requires BA/BS in a health related field and minimum of 5 years of clinical experience.
• Current, unrestricted RN license in applicable state(s) required.
• Multi-state licensure is required if this individual is providing services in multiple states.
• Certification as a Case Manager is preferred.

Responsibilities

• The Nurse Case Manager II is responsible for care management for members with complex and chronic needs.
• Assess, develop, implement, coordinate, monitor and evaluate care plans across the care continuum.
• Coordinate authorizations/referrals within benefits structure or through extra-contractual arrangements.
• Interface with Medical Directors and Physician Advisors on treatment plans; discharge planning.
• Perform duties telephonically or on-site such as at hospitals for discharge planning.
