Qualifications

• 3+ years’ clinical practice including patient care coordination and health assessments.
• RN with current unrestricted KY licensure.
• 2+ years’ experience using MS Office and multiple systems.
• Willingness to travel 80-90% in-state; mileage reimbursed.
• Flexibility to work beyond standard business hours.

Responsibilities

• Assess member needs and coordinate care plans across services.
• Apply clinical judgment to reduce risk factors and barriers.
• Collaborate with care partners and participate in case conferences.
• Ensure compliant use of case management processes and documentation.
