Qualifications

• 3+ years' clinical practice experience including patient care coordination and health assessments
• RN with current unrestricted state licensure in KY
• 2+ years' experience using personal computer, keyboard navigation, navigating multiple systems and applications; and using MS Office Suite applications (Teams, Outlook, Word, Excel)
• Must possess reliable transportation and be willing and able to travel in-state up to 80-90% of the time
• Flexibility to work beyond core business hours Monday-Friday, 8am-5pm CST
• Candidate must reside in one of the KY service region counties listed in the ad

Responsibilities

• Assess members through regular in-person or telephonic contact to evaluate medical and psychosocial needs
• Develop and revise care plans to address biopsychosocial needs
• Coordinate case management activities and liaise with care partners
• Review prior claims to assess impact on eligibility and care
• Utilize motivational interviewing to engage members
• Ensure documentation and regulatory compliance
