Job summary

CVS Health is seeking a Case Manager Registered Nurse in Owensboro, KY. You will assess members’ medical and psychosocial needs, plan and coordinate care, and support wellness through field-based case management.

The role requires an RN with 3+ years of clinical practice, strong data review skills, and experience in care coordination. In-state travel is heavy, and a KY license is mandatory.

Qualifications

• Associates degree in Nursing or Diploma RN required.
• BSN preferred.

Responsibilities

• Assess members' needs through clinical evaluation and data review.
• Develop and implement member care plans addressing biopsychosocial needs.
• Conduct comprehensive evaluations of member eligibility and benefit plans.
• Utilize motivational interviewing to engage members in their healthcare decisions.
• Consult with care partners to overcome barriers and present cases at conferences.
• Monitor and revise care plans based on member progress and needs.
• Ensure compliance with regulatory and company policies in case management processes.
• Travel to meetings and training sessions as required.
