Job summary

Qualifications

• Proficiency with MS Office and clinical documentation systems.

Responsibilities

• Assess member needs and eligibility; plan, implement and coordinate case management activities.
• Develop proactive care plans; address barriers to improve short and long-term outcomes.
• Collaborate with care partners; present cases at conferences for multidisciplinary input.
• Travel to field locations; document activities and ensure regulatory compliance.
