Responsibilities

• Coordinate plan of care with the health care team to improve quality and efficiency.
• Review admissions and assess discharge planning requirements within one business day of admission.
• Identify delays in service and work with patients, families, and care teams to ensure timely action.
• Collaborate with home care providers for benefits and coverage of home care services.
• Provide ongoing consultation and training to staff on discharge and home care issues.
