Job summary

CVS Health is seeking a skilled Care Management RN to join our team in delivering patient-centered, compliant case management across the care continuum. You will collaborate with members, families, employers, providers and insurers to coordinate comprehensive plans and improve outcomes.

In this full-time role, you will assess needs, navigate Medicare requirements, and communicate clearly with diverse stakeholders while adhering to regulatory guidelines.

Qualifications

• Active, unrestricted Compact RN license in state of residence.
• 2+ years of care management experience.
• 2+ years of Medicare experience.

Responsibilities

• Act as liaison with members, families, employers, providers, and insurers.
• Coordinate case management for catastrophic and chronic cases across care continuum.
• Interact with members telephonically or in person, including home visits if needed.
• Assess medical/vocational status and develop a plan of care to optimize outcomes.
• Communicate with stakeholders including providers, attorneys, and carriers.
• Prepare documented case work activities and maintain records.
• Consult with internal multidisciplinary team to maximize health outcomes.
• Ensure compliance with laws and regulations governing rehabilitation services.
