Job summary

CVS Health is seeking a Case Manager Registered Nurse to develop proactive care plans that improve member outcomes and overall wellness for dual eligible populations. The role requires a NJ RN license, 3+ years clinical experience, and 2+ years in care management or related fields.

Travel up to 50% and Middlesex County proximity preferred. The position offers full-time hours, standard business schedule, and benefits including medical, dental, and vision coverage, PTO, and retirement options.

Qualifications

• Minimum 3+ years of clinical practice experience.
• Minimum 2+ years in Care Management, Discharge Planning, and/or Home Health Care Coordination preferred
• Active and unrestricted RN licensure in the state of NJ required
• Certified Case Manager preferred
• NJC Certification preferred but not required

Responsibilities

• Develop proactive plans of care to enhance member outcomes.
• Conduct evaluations of member needs using clinical tools and data review.
• Apply clinical judgment to reduce risk factors and address health indicators.
• Conduct assessments considering information from various sources.
• Use a holistic approach to assess the need for referrals to clinical resources.
• Collaborate with supervisors and stakeholders to overcome barriers.
• Present cases at interdisciplinary case conferences.
• Utilize case management processes in compliance with regulations.
• Employ motivational interviewing skills for member engagement.
• Coordinate all case management activities to evaluate medical needs.
