Job summary

CVS Health in Michigan seeks an experienced RN Case Manager to elevate patient care through community-based coordination. You will work with members, providers and families to assess needs, develop plans, and facilitate appropriate services while maintaining HIPAA compliance.

The role supports work-from-home arrangements, requires Michigan RN license, an associate degree (bachelor preferred), and 3+ years of clinical and care-management experience; bilingual Spanish is a plus.

Qualifications

• Active nursing license in Michigan (RN).
• 3+ years clinical experience with case management preferred.
• Experience coordinating home health and chronic care.
• Proficient in standard software including Word, Excel, Outlook, PowerPoint.

Responsibilities

• Coordinate case management for catastrophically ill or complex cases.
• Interact with members, families, providers, and insurers to develop care plans.
• Assess medical and psychosocial needs to optimize wellness and outcomes.
• Document activities and communicate with stakeholders.
• Collaborate with internal teams to meet regulatory requirements.
