Job summary

CVS Health is seeking a remote Community Care Case Manager to coordinate care for Medicare members. You’ll assess, plan and facilitate services, communicating with doctors, insurers and families to optimize health outcomes.

Required a valid RN license and 3+ years of clinical practice with Medicare. 2+ years in case management or home health is preferred, with bilingual Spanish a plus. The role supports a remote, work-from-home model with US-wide eligibility.

Qualifications

• 3+ years clinical practical experience (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members
• 2+ years case management, discharge planning and/or home health care coordination experience
• Bilingual - Spanish (preferred)

Responsibilities

• Acts as liaison with member/client/family, employer, providers, insurers and healthcare personnel as appropriate
• Implements and coordinates case management activities across continuum of care
• Interacts telephonically or in person with members/clients and their homes, worksites or physicians' offices
• Develops care plans to facilitate wellness, return to baseline and optimal functioning
• Monitors progress and documents case work; communicates with stakeholders
• Adheres to laws, regulations, and HIPAA guidelines; participates in education and prevention information
