Qualifications

Responsibilities

• Conduct comprehensive telephonic and face-to-face assessments.
• Develop and implement personalized care plans for short- and long-term wellness.
• Coordinate care across providers and services for a seamless member experience.
• Identify risks and gaps in care using clinical data insights.
• Support members with complex conditions using a holistic approach.
• Facilitate referrals to clinical and community resources.
• Participate in case conferences with multidisciplinary teams.
• Review claims history and benefit eligibility to inform planning.
• Engage members to promote active participation in care.
• Ensure all activities comply with guidelines.
