Qualifications

• Active and unrestricted RN licensure in Illinois.
• 3+ years of clinical practical experience in diabetes, CHF, CKD, post-acute care, hospice or related areas with Medicare.
• 2+ years in case management, discharge planning or home health coordination.
• Proficient with standard office software and healthcare systems.

Responsibilities

• Serve as liaison with members, employers, providers, and insurers.
• Coordinate case management for catastrophic and chronically ill members across the care continuum.
• Interact with members telephonically or in person.
• Meet with members at home, worksites, or physician offices for ongoing care.
• Assess medical and vocational status; develop a plan to optimize outcomes.
• Document case work and communicate with stakeholders.
• Collaborate with internal multidisciplinary teams to maximize outcomes.
• Provide education and prevention information related to care.
