Qualifications

• Resident of Florida with an active and unrestricted Florida RN license.
• 3+ years clinical experience in areas such as diabetes, CHF, CKD, post-acute care, hospice or palliative care with Medicare members.
• 2+ years of case management, discharge planning and/or home health care coordination experience.

Responsibilities

• Act as liaison with members, providers, and insurers to coordinate care.
• Coordinate case management activities for complex and chronically ill patients.
• Conduct telephonic or in-person interactions to assess needs and plan care.
• Develop and document care plans to promote wellness and return to work.
• Communicate with clinicians and stakeholders to facilitate services and approvals.
• Maintain documentation of case work and follow regulatory requirements.
