Qualifications

• Active and unrestricted Compact RN license in one of the required states.
• 3+ years of clinical practical experience, preference in chronic/complex care.
• 2+ years in case management, discharge planning, or home health coordination.
• Proficient with Microsoft Word, Excel, Outlook and PowerPoint.

Responsibilities

• Coordinate case management activities for diabetic and chronically ill members.
• Interact with members and healthcare providers to plan and monitor care.
• Prepare documentation of case work and coordinate referrals as needed.
• Communicate with stakeholders to facilitate appropriate care plans.
• Ensure compliance with regulatory and company policies in case management.
