Qualifications

• Active and unrestricted Compact RN licensure in FL or OH.
• 3+ years of clinical experience with Medicare members.
• 2+ years in case management, discharge planning, or home health care coordination.
• Proficiency with Microsoft Word, Excel, Outlook and PowerPoint.
• Ability to work in a fast-paced, high-volume environment.

Responsibilities

• Acts as a liaison with member/client/family, employer, provider(s), insurance companies, and healthcare personnel.
• Implements and coordinates all case management activities for catastrophic and chronically ill members across the continuum of care.
• Interacts with members by telephoning or in person; may visit homes or physician offices for ongoing case management.
• Assesses medical/vocational status and develops a plan of care to optimize wellness and outcomes.
• Documents all case work activities and communicates with stakeholders.
• Collaborates with internal multidisciplinary teams to maximize health outcomes.
• Conducts outreach to treating physicians or specialists as needed.
