Job summary

CVS Health is seeking a Community Care Case Manager to coordinate case management activities, assess medical and vocational needs, and facilitate optimal outcomes for members through collaboration with providers, employers and insurers.

The role requires an Illinois RN license, 3+ years clinical experience with 2+ years in case management, and strong communication skills. A Bachelor’s degree is preferred, and travel may be required. Work-from-home enabled with a comprehensive benefits package.

Qualifications

• Active and unrestricted Illinois RN license is required.
• 3+ years clinical experience; 2+ years in case management, discharge planning, or home health coordination.
• Bilingual in Spanish preferred; strong communication and organizational skills.

Responsibilities

• Coordinate case management activities for catastrophic and chronically ill members across the continuum of care.
• Assess medical and vocational status; develop care plans to facilitate appropriate condition management and return to baseline or work.
• Communicate with members, providers, employers, insurers and other stakeholders to coordinate care.
• Prepare documentation of case work activities and ensure regulatory compliance (HIPAA).
