Job summary

CVS Health is seeking an RN Case Manager to join Community Care. You will coordinate and advocate for patient care, collaborating with providers to address needs along the care continuum.

Qualified candidates hold an active Compact RN license in Ohio or Florida, with 3+ years of clinical experience and 2+ years in case management or home health. This full-time remote role offers comprehensive benefits and requires residency in OH or FL.

Qualifications

• Active and unrestricted Compact RN licensure in their state of residence (OH or FL).
• 3+ years of clinical practical experience with Medicare members (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac, Home Health).
• 2+ years of case management, discharge planning and/or home health care coordination experience.
• Proficiency with Microsoft Word, Excel, Outlook and PowerPoint, and some proprietary applications.
• Able to work in a fast-paced high-volume environment with strong time management and prioritization.

Responsibilities

• Coordinate and facilitate comprehensive care across providers.
• Advocate for patients with effective communication and resource utilization.
• Develop personalized care plans and monitor patient progress for positive outcomes.
• Outreach to treating physicians or specialists as needed.
• Prepare documentation of case work activities and collaborate with the multidisciplinary team.
