Qualifications

• RN Compact license required.

• Minimum 2+ years discharge planning and/or home health care coordination.

Responsibilities

• Facilitate delivery of benefits and healthcare information to determine eligibility.
• Develop and support Health Strategies, policies, and programs for member wellness and return to work.
• Coordinate care through assessment, planning, facilitation, and advocacy to meet health needs.
