Job summary

CVS Health in New Jersey is seeking a Registered Nurse Case Manager to join our Community Care team. You will coordinate care for members, collaborate with providers, insurers and community resources, and help patients address medical and social needs.

This full-time role requires an active NJ RN license, 3+ years clinical experience, 2+ years in case management or home health coordination, and ability to travel up to 10% within the region. Schedule is Monday–Friday, 8:00 AM–5:00 PM ET.

Qualifications

• Active and unrestricted NJ RN Compact License
• 3+ years clinical practice experience
• 2+ years case management, discharge planning, and/or home health care coordination experience
• Ability to travel up to 10% within designated geographic area
• 2+ years proficiency with standard Microsoft Office applications (Word, Teams, Excel, Outlook, PowerPoint)

Responsibilities

• Act as liaison with members, providers, insurers, and healthcare personnel
• Coordinate case management for catastrophic and chronic cases
• Interact with members telephonically or in person
• Assess medical status and develop plans to optimize wellness and outcomes
• Communicate with stakeholders and document activities
• Collaborate with multidisciplinary team to maximize outcomes
• Provide education and prevention information
• Ensure compliance with applicable regulations
• Assess benefits eligibility and navigate care integration
• Monitor progress toward desired outcomes
