Mass Digital Health is searching for a full-time Community Care Nurse to enhance patient care. This role is critical in utilizing best-in-class clinical models to impact community health positively.

Applicants must hold active RN licensure and have experience in case management and care coordination. The position offers competitive pay ranging from $60,522 to $129,615 annually, alongside a comprehensive benefits package.

Formación

• Must have active and unrestricted Compact Registered Nurse (RN) Licensure.
• 3+ years of clinical experience with Medicare members preferred.
• 2+ years of case management and home health care coordination experience.

Responsabilidades

• Acts as a liaison with various stakeholders regarding member care.
• Implements case management activities for chronically ill members.
• Assesses member/client needs and develops care plans.

Conocimientos

Active RN Licensure in multiple states

3+ years of clinical practical experience

2+ years of case management experience

Proficiency in Microsoft Office

Strong time management skills

Educación

Associate’s Degree in Nursing

Bachelor’s Degree in Nursing
