Qualifications

• Must have active and unrestricted Compact Registered Nurse (RN) Licensure.
• 3+ years of clinical experience with Medicare members preferred.
• 2+ years of case management and home health care coordination experience.

Responsibilities

• Acts as a liaison with various stakeholders regarding member care.
• Implements case management activities for chronically ill members.
• Assesses member/client needs and develops care plans.

Skills

Active RN Licensure in multiple states

3+ years of clinical practical experience

2+ years of case management experience

Proficiency in Microsoft Office

Strong time management skills

Education

Associate’s Degree in Nursing

Bachelor’s Degree in Nursing
