Mass Digital Health is seeking a Case Manager to lead care coordination across hospital levels. This role requires an accredited RN with at least 2 years of clinical patient care experience. Responsibilities include medical necessity documentation, discharge planning, and leading morning huddles.

The ideal candidate will possess excellent communication skills, strong judgment, and flexibility in handling diverse situations. Competitive compensation package offered, including a minimum of USD $59.94 and a maximum of USD $82.42.

Qualifications

• At least 2 years clinical patient care experience on a medical surgical unit.
• Licensed RN in the State of California; certification in Case Management or Utilization Review required.
• Proficiency with computers and electronic forms of communication.

Responsabilités

• Completes medical necessity criteria for both inpatient and outpatient visits.
• Leads morning huddles focusing on patient status and discharge planning.
• Conducts readmission interviews with readmitted patients.

Formation
