Job summary

WellMed in Austin is seeking an RN Case Manager to support high-risk patients in navigating the healthcare system within an in-office setting. You will collaborate with clinicians to optimize care and patient experience.

Responsibilities include health assessments, medication reconciliation, motivational interviewing, patient education, and referrals. You will document in the EMR, participate in PCC meetings, and help maintain continuity of care for chronic conditions.

Qualifications

• Bachelor's or Associate degree in nursing required.
• Active RN license in the state of employment.
• Current BLS certification.
• 2+ years in physician's office, clinical, or hospital setting.
• Knowledge of chronic diseases (COPD/diabetes/CHF/IHD).
• Proficient with electronic medical records.
• Strong interpersonal and teamwork abilities.
• TB screening and immunity documentation required.

Responsibilities

• Identify high-risk patients with providers and clinic staff.
• Assess health conditions and perform medication reconciliation.
• Coordinate transitions from inpatient to outpatient care.
• Provide patient education and referrals to services.
• Document accurately in the electronic medical record.
• Participate in PCC meetings and weekly reports.
• Maintain current nursing practice and evidence-based knowledge.
• Perform additional duties within scope as needed.
