Job summary

WellMed, part of Optum, is seeking a Registered Nurse to join our Leander, TX team as a Healthcare Manager focused on high-risk patients. You will collaborate with physicians, nurses, pharmacists, and social workers to navigate care, perform assessments, and empower patients toward better outcomes.

The role requires a relevant nursing degree, active RN license, BLS, and 2+ years in clinical/hospital or physician office settings. On-site position with comprehensive benefits.

Qualifications

• Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience.
• Registered Nurse with an active and unrestricted license to practice in the state of employment.
• Current BLS certification.
• 2+ years of experience in a physician's office, clinical or hospital setting.
• Experience interacting productively with individuals and multidisciplinary teams.
• Knowledge of chronic diseases, especially COPD, asthma, diabetes, CHF and IHD.
• Proficient computer skills to work efficiently with electronic medical records.

Responsibilities

• Identify patients at high risk in collaboration with providers and clinic staff.
• Support longitudinal care for patients with chronic conditions through assessments and education.
• Coordinate transitions from inpatient to outpatient settings and manage contingency planning.
• Provide patient education and create referrals to appropriate resources.
• Document accurately in the electronic medical record and participate in PCC meetings.
• Prepare reports for weekly meetings and support continuous nursing competence.
