Job summary

Klamath Tribal Health is seeking an RN Case Manager to work collaboratively with providers to improve care for patients with chronic conditions at the Wellness Center in Klamath Falls. The role involves education, care coordination, and resource management, with a focus on cost-effective, high-quality care.

The position requires an active RN license, ACLS certification, 3+ years in ambulatory/outpatient settings, and a BSN.

Qualifications

• Current RN license; Oregon license required within 90 days of hire.
• ACLS certification is required.
• Basic computer skills, EHR experience.
• BSN required; degree verification.
• Three years’ experience in ambulatory/outpatient settings; case management experience preferred.
• Background check and TB skin testing required.
• Flexible schedule; Indian Preference will apply.

Responsibilities

• Identify patients who meet criteria for care management and coordinate care.
• Establish individualized care plans with patients and caregivers; monitor progress.
• Educate patients and families to enable self-management and optimize outcomes.
• Coordinate with care managers and community resources for services and referrals.
• Maintain complete, accurate documentation and ensure timely data entry and reporting.
