Job summary

Oak Street Health is seeking an experienced RN, Case Manager to lead care coordination for complex patients in a field-based, on-site role across Bethlehem and surrounding communities.

You will collaborate with PCPs, social workers, and interdisciplinary teams to prevent avoidable admissions and ensure smooth transitions of care, while educating patients in chronic condition management and self-management strategies.

Qualifications

• Current RN license in the assigned state is required.
• Bachelor degree in nursing preferred.
• 6-8 years nursing experience.
• Certified Case Manager (CCM) required, or willingness to obtain within 12 months of hire.
• 2+ years experience in transitional nursing, ER nursing, care coordination, discharge planning, or home health is strongly preferred.
• Demonstrated skill in motivational interviewing, patient activation, time management, and navigating community and social resources.
• A flexible and positive attitude; comfortable with ambiguity and change.
• High emotional intelligence and ability to evaluate situations from multiple perspectives.
• Access to reliable transportation and ability to travel throughout OSH communities.
• US work authorization.

Responsibilities

• Manage an assigned caseload of complex patients to reduce admissions and medical utilization.
• Own panel metrics in admission prevention, readmission prevention, and transitions of care.
• Coordinate care across the PCP, Social Worker, and care team members.
• Lead in-person interdisciplinary care planning meetings for coordinated care.
• Educate patients and families, empowering them in their care and advocating for needs.
