Job summary

Premise Health is seeking a Full Time RN Case Manager for our Health Center in Pella, IA. You will work with client employees and dependents, developing personalized care plans and coordinating with a multidisciplinary team to promote prevention and wellness.

Ideal candidates will have an Iowa RN license, BLS certification, and 3+ years of RN experience in chronic condition management. BSN preferred, with a focus on patient advocacy and education within Epic EMR systems.

Qualifications

• Licensed Registered Nurse in state of practice (Iowa)
• Minimum three years RN experience caring for members with chronic conditions is preferred
• Current BLS/ACLS or health care provider CPR certification preferred
• BSN or higher degree preferred; certifications in case management are a plus

Responsibilities

• Develops personalized care plans using provider plans and member preferences
• Supports a multidisciplinary team approach focused on prevention and education
• Establishes supportive relationships with members and documents progress
• Assesses educational/clinical/social needs and prioritizes care
• Coordinates with clinical teams to arrange further treatment when needed
• Delivers self-care education and promotes wellness programs
• Maintains member privacy and confidentiality
• Acts as member advocate and Care Team facilitator
