Job summary

Southern Indian Health Council is seeking a Registered Nurse Case Manager to deliver Care Management services for patients with chronic illnesses. You will be responsible for managing patients, connecting them with professional services, and collaborating with a multidisciplinary team.

The ideal candidate must have a valid California RN license, at least 3 years of relevant experience, and strong team-building and organizational skills. The position offers a salary ranging from $97,160 to $135,496 annually, depending on experience.

Qualifications

• Must possess a current California Registered Nurse license.
• A minimum of 3 years’ experience in a similar healthcare role is required.
• Must have knowledge of HIPAA in the ambulatory care setting.

Responsibilities

• Deliver Care Management services to the SIHC population.
• Manage patients with chronic illness and life-altering conditions.
