Job summary

Klamath Tribal Health is seeking an RN Case Manager to work with the Wellness Center team to improve health outcomes for patients with chronic conditions. The role involves patient education, coordination of services, and balancing clinical quality with cost management, with interactions in person or via telecommunication.

Responsibilities include care planning, documentation, and collaboration with providers, with emphasis on prevention and smooth transitions of care within the tribal health

Qualifications

• Current RN license required and ACLS certification.
• Bachelors degree in nursing preferred; 3+ years of ambulatory/clinic experience is desirable.
• Proficiency with electronic health records and data entry.

Responsibilities

• Coordinate care with providers and care managers to support patients with chronic conditions.
• Educate patients and families to manage health care needs.
• Document all patient contacts and maintain accurate health records.
• Assess barriers, readiness to change and coordinate with community resources.
• Assist in reporting quality measures and preventive care tracking.
