Job summary

Full Circle Health is seeking a Clinical Case Manager to coordinate care for patients with chronic or complex conditions at the Raymond Clinic. You will work with providers, the interdisciplinary team, patients and community partners to ensure comprehensive care planning and delivery.

Requirements include an ADN/BSN or MSW, at least two years of clinical practice, and Idaho licensure with BCLS. CCM certification is preferred; pediatrics experience is a plus.

Qualifications

• A.D.N. or B.S.N. from an accredited College of Nursing, or a Masters in Social Work from an accredited university.
• Minimum of two years of clinical practice; population health, case management or transitions of care experience preferred.
• Licensed to practice as an RN or LPN in Idaho, or MSW with licensure likely, current BCLS certification; CCM preferred.
• Pediatrics experience is preferred but not required.

Responsibilities

• Coordinate care for individuals with chronic or complex conditions.
• Collaborate with health care providers, interdisciplinary team, patients and community health partners.
• Identify, screen, assess, plan, implement, coordinate, monitor and evaluate health services.
• Work within 8:00am–5:00pm, Monday–Friday schedule.
