Job summary

Mercy is seeking a Care Manager who will join an interdisciplinary team to assess, plan, advocate, and coordinate patient care from admission through discharge. You will help ensure a safe transition after hospitalization while aligning with Mercy's mission, values, and service standards.

The role emphasizes effective transition planning, length of stay oversight, patient safety, and quality care using performance metrics and best practices.

Qualifications

• Graduate of an accredited School of Nursing (RN) required.
• BLS (CPR) required at hire or within 90 days.
• Current state license in employment state required.
• Care Management or Utilization Management experience preferred.
• Certification in Case Management preferred.

Responsibilities

• Assess, plans, advocates, and coordinates care from admission to discharge.
• Ensure safe transition post hospitalization and oversee care with the Mercy model.
