Job summary

The Staff Pad is partnering with a respected healthcare organization in Hamilton to recruit an experienced RN Case Management Manager who leads patient care transitions and champions safe, efficient discharge planning.

You will oversee a high-performing Case Management team, collaborate with physicians, nurses and social work to remove barriers to timely discharge, ensure regulatory compliance, and advocate for patients throughout the continuum of care.

Qualifications

• Graduate of an accredited School of Nursing.
• RN license in Texas; CPR/BLS current; ACLS preferred.
• 5+ years of registered nurse experience; case management leadership experience preferred.

Responsibilities

• Conduct discharge planning and timely discharge assessments.
• Lead and coordinate case management activities from admission to discharge.
• Collaborate with physicians, nursing staff, and social work to remove discharge barriers.
• Ensure compliance with regulatory requirements and utilization management policies.
• Educate patients and families; liaison among patients, families, and care teams.
