Responsibilities

• Facilitates communications among patient, family, and care teams to resolve barriers to timely care.
• Collaborates with primary care, hospital teams, and post-acute managers to ensure quality, cost-effective services.
• Uses evidence-based approaches to activate patients and set SMART goals for self-management.
• Identifies barriers to learning and care delivery and assists in planning.
• Manages CM caseload independently and ensures timely documentation for regulatory requirements.
• Coordinates referrals and patient information across facilities for smooth transitions of care.
