Job summary

TriStar Hendersonville Medical Center, part of HCA Healthcare, seeks an experienced RN Case Manager to coordinate care, manage length of stay, and develop a safe discharge plan with the multidisciplinary team.

Requirements include RN license, ADN required with BSN preferred, and 2+ years in case management or 3+ years in clinical nursing; InterQual experience preferred. This role focuses on patient-centered care and optimal resource utilization.

Qualifications

Responsibilities

• Provides case management services for inpatient and observation patients.
• Identifies patients at risk during transitions between care settings.
• Performs comprehensive psychosocial, medical, and discharge assessments.
• Reassesses clinical condition and plans mitigation strategies.
• Coordinates care plan and drives discharge planning with the care team.
• Collaborates with Social Services for post-acute needs.
• Refers patients to third-party payer and disease management programs.
• Documents recommendations and communicates with the health care team.
• Promotes safe discharge and appropriate resource utilization.
