Job summary

Baptist is seeking an RN to manage care for a specific population, ensuring safe transitions and appropriate utilization of resources across the continuum of care. You will perform early assessments during hospitalization and support compliance with payer requirements.

Ideal candidates have 1 year of clinical experience; 3 years in case management in hospital or payer settings is preferred, with BSN/MSN preferred and CCM certification as a plus.

Qualifications

• RN with at least 1 year of clinical experience.
• RN with 3 years of clinical experience in case management preferred in hospital or payer setting.
• Diploma or Associate Degree in Nursing is required.
• BSN or MSN preferred.
• Certified Case Manager (CCM) preferred.
