Qualifications

• Associate Degree Nursing is required.

• 1-3 years Case Management experience.
• 1-3 years clinical healthcare experience (acute or hospital).

Responsibilities

• Coordinate patient-centered care and transitions.
• Assess readmission risk factors and barriers to care.
• Develop transition plans in collaboration with patient/family and care team.
• Communicate with physicians, care team, patients and families.
• Plan activities to meet patient needs and optimize resources.
• Advocate for safe, timely discharge and follow-up.
• Maintain accurate documentation and case tracking.
• Perform additional duties to support goals.
