UAB Medicine is seeking a Registered Nurse Care Coordinator to support both inpatient and ambulatory settings, focusing on post-discharge needs and referrals for home care. You will work within a Magnet-designated team to ensure high-quality, coordinated patient care.

The role emphasizes care coordination across disciplines, screening for post-discharge needs, and contributing to the care plan with interdisciplinary colleagues. A clinical ladder supports professional growth and ongoing education.

Qualifications

• Bachelor's or Associate's degree in Nursing required.
• RN licensure by the Alabama Board of Nursing required or eligible.
• BLS certification required or to be obtained within 30 days of employment.
• ACLS and/or PALS may be required depending on unit.

Responsibilities

• Coordinate, negotiate and obtain services and resources for patient care.
• Review case criteria to determine medical necessity for admission and continued stay.
• Provide discharge planning and care coordination identifying cost-effective care options.
