Job summary

Parkridge Medical Center in Chattanooga, TN, part of HCA Healthcare, is seeking an RN Case Manager to coordinate the care plan and ensure safe transitions for patients across inpatient and observation services.

The role requires an active RN license, a nursing degree, and experience in case management or clinical nursing. InterQual experience is preferred. This position emphasizes collaboration with Social Services and a patient/family-centered approach.

Qualifications

• RN license required.
• Associate Degree in Nursing or Nursing Diploma required.
• Bachelor’s Degree in Nursing preferred.
• 2+ years’ experience in case management OR 3+ years in clinical nursing required.
• InterQual experience preferred.

Responsibilities

• Provides case management services for inpatient and observation patients.
• Identifies patients at risk during transitions between care levels.
• Performs psychosocial, medical and discharge assessments with available resources.
• Reassesses clinical condition and strategies to mitigate readmission risk.
• Coordinates care plan and drives discharge planning with the care team.
• Partners with Social Services to ensure post-acute needs are met.
• Refers timely to Social Services when psychosocial risk factors are identified.
• Involves patient and family in identifying needs and goals.
• Evaluates progression of care using InterQual criteria.
• Makes referrals to third-party payers and disease/case management programs.
• Facilitates patient throughput with focus on quality and efficiency.
• Documents recommendations, discharge plans, and care coordination activities.
• Aligns needs with resources to ensure a safe discharge/transition.
• Adheres to Code of Conduct and Mission and Value Statement.
