Qualifications

Responsibilities

• Oversees overall patient care from start to end of life.
• Performs initial, comprehensive, and ongoing skilled nurse visits.

• Coordinates interdisciplinary care and supervises total patient care.
• Administers medications as prescribed.
• Documents all treatment, assessments, and patient education.
• Maintains effective communication with patients, families, physicians and staff.
• Participates in on-call rotations and case conferences.
