AccentCare, Inc. is seeking a Registered Nurse Case Manager to oversee patient care plans from admission through discharge in Oakdale and surrounding areas. The role combines direct and indirect care and coordinates with RNs, LPNs, and CHHAs to ensure quality outcomes.

The ideal candidate has RN licensure in the state, at least one year of RN experience, and CPR certification. Compensation reflects per-point pay with annualized estimates; base schedule is Monday–Friday, 8:00 a.m. – 5:00 p.m.

Qualifications

• RN licensed in the state of operation.
• Minimum 1 year of RN experience.
• Current CPR certification required.

Responsabilités

• Assess nursing needs and manage the patient's care plan.
• Complete initial patient assessment at admission with history.
• Identify nursing diagnoses and assess medicines for adverse reactions.
• Set realistic, measurable goals aligned with diagnosis.
• Inform physician of assessments and interventions.
• Implement nursing interventions within scope of practice.
• Re-evaluate patient needs as required.
• Document assessments, progress, and discharge notes.
