Job summary

AccentCare, Inc. is seeking a Registered Nurse Case Manager to manage patients' care plans from admission through discharge and to ensure the delivery of quality patient care. You will direct staff and coordinate with interdisciplinary teams to meet each patient's goals in the Oakdale area.

The role combines direct and indirect patient care with schedule of Monday–Friday, 8:00 a.m.–5:00 p.m. Pay is estimated at $87,000–$102,000 annually, paid on a per-point basis as noted in the posting.

Qualifications

• Graduate from an approved school of professional nursing and licensed as RN in the state of operation.
• Minimum of one year of experience as an RN.
• Current CPR Certification required.

Responsibilities

• Assess nursing needs and manage the patient's care plan.
• Complete initial patient assessment at admission with history.
• Select appropriate nursing diagnosis for the patient.
• Assess medications for ineffective therapy or adverse reactions.
• Establish realistic, measurable goals aligned with diagnosis.
• Inform physician of assessment results and interventions.
• Implement nursing interventions within the nurse practice act.
• Re-evaluate patient needs and document progress and discharge notes.
