Job summary

Tenet Healthcare is seeking an RN Case Manager to coordinate care across the continuum, ensuring medical necessity, appropriate level of care, and timely discharge planning. You will work with physicians, families, and the care team to optimize resource use and patient outcomes.

The role emphasizes education, transition management, and collaboration with interdisciplinary teams to prevent readmissions and support post-acute needs in a fast-paced hospital environment.

Qualifications

• Graduate of an accredited school of nursing.
• Bachelor's or master's degree in nursing preferred.
• 2 years of acute hospital patient care experience required.
• Acute hospital case management experience preferred.
• RN licensure in state of practice.
• Accredited Case Manager (ACM) preferred.

Responsibilities

• Accurately screen medical necessity and submit for Physician Advisor review.
• Coordinate care and transition planning.
• Lead multidisciplinary patient care conferences.
• Identify complex patients for Social Work Services.
• Maintain timely documentation in Tenet Case Management system.
• Ensure accurate patient demographics and insurance information.
• Identify and report avoidable days and utilization.
• Provide patient and family education about plan of care.
