Job summary

Tenet Healthcare’s RN Case Manager in Detroit, MI leads care coordination to ensure appropriate utilization, safe discharge, and timely throughput across the care continuum. You will assess needs, coordinate transitions, and guide patients and families through the plan of care.

The role requires clinical excellence in medical necessity screening, collaboration with physicians and payers, and proactive education to patients and caregivers, ensuring compliance with Tenet policies and standards.

Qualifications

• Graduate from an accredited school of nursing; a BSN or related degree or equivalent experience.
• Two years of acute hospital patient care experience; acute hospital case management experience preferred.
• License to practice as a Registered Nurse in Michigan; ACM preferred and InterQual education/certification requirements within 30 days of hire.

Responsibilities

• Accurate medical necessity screening and submission for Physician Advisor review.
• Care coordination and transition planning assessment and reassessment.
• Lead and facilitate multi-disciplinary patient care conferences; referrals to Social Work; documentation in Case Management system.

Education

Registered Nurse license in Michigan

Bachelor’s degree in Nursing or related field

Accredited Case Manager (ACM) preferred
