Job summary

Sheridan Memorial Hospital is seeking an RN Case Manager to coordinate patient care across the continuum, ensuring timely, appropriate transitions and regulatory compliance. The role requires collaboration with physicians, nursing, social work, and post-acute providers to optimize outcomes and resource use.

The nurse will lead discharge planning, monitor patient status, and support payer interactions while promoting patient-centered, high-quality care.

Qualifications

• RN with case management experience preferred.
• Experience in discharge planning and transitions of care.
• Knowledge of CMS/Medicare requirements and utilization management.

Responsibilities

• Perform comprehensive case management assessment of assigned patients and develop individualized transition and discharge plans.
• Coordinate referrals and transitions to post-acute services including home health, hospice, swing bed, SNF, and outpatient services.
• Support utilization review activities and ensure compliance with CMS requirements and hospital policy.
• Collaborate with physicians, nursing, social work, rehabilitation, pharmacy, and payers to optimize patient outcomes.
• Document progress and communicate changes in patient status to the care team.
